This is in reference to UNICEF USA “Save A Child” ad campaigns.

There’s a (complicated) history of white Americans looking down on Americans of other ethnicities, especially blacks, from their seat of delusional racial or ethnic superiority.
The Antebellum slave era, the Jim Crow era, and the “Post-racial Era” have all had their white bigots.

In the olden days (you can check The Bible), people with the disease called Leprosy, which causes skin sores, nerve damage, and muscle weakness that gets worse over time. People with Leprosy used to be chastised and mocked by being called lepers because people believed that God was punishing Lepers. People also never touched lepers because they were afraid that they might too contract Leprosy if they touched a leper.

A shot to the former member of GOOD Music, Consequence, who has been dissing Pusha (most notably on The Plagurist Society (Pusha T Diss)), claiming he has been biting his lyrics. Pusha doesn’t want to stoop down to Cons level, as Cons has also been dissing GOOD founder Kanye West

Some will think this is a shot at Drake, or Ambassador Rick (who actually dissed Pusha in a song).
Pusha’s reminding listeners that although he’s new as a solo artist, his time as one half of The Clipse grants him veteran status.

Worth noting that the flow and rhyme scheme vaguely resemble both Ye and Jay’s verses on So Appalled.
Might not be a coincidence because Pusha was also featured on that song and this song is also called So Obvious.
There are two meanings for this line:
The “K-I-L” sets up the line like Push is going to spell “kill”, and then he exclaims “oh” as if he found the “L”,
The “K-I-L” is finished by “O” (Oh) to spell “kilo” – of course in reference to cocaine – which is then confirmed by the “triple beam measure” lyric.
